Suggestions:

– create a SYLLABUS TEMPLATE for all courses

-required all faculty members to utilize the template and maintain up-to-date syllabi

-perform and document program-wide audits

Common citations include:

– missing supervisor/intern signatures, date of treatment

-missing forms (i.e. intake, privacy disclosure, etc)

-intake form fields left blank

-incomplete or minimal documentation of intake, assessment, treatments

-insufficient privacy protections

-off-site clinics not adhering to some standards
